If you have concerns or comments about patient care that have not been addressed by the department where the care is being received (or was received), please feel welcome to contact the Patient Relations Office at 519-372-3920 ext 2311. The Patient Relations office hours of operation are 8:00 am to 4:30 pm, Monday to Friday. A confidential voice mail is available that allows you to leave your name and contact number and a representative from Patient Relations will call you back on the next business day.

We also welcome your comments and value your opinion, suggestions and general information questions anytime. Comments concerning the usefulness of the website information, how it is presented and additional information you would like to see on the site can be submitted to Provide Feedback.
