Director, Clinical Integration

Owen Sound

Reporting to the VP Patient Care and Quality, the Director of Clinical Integration provides leadership in the development and implementation of a corporate vision and ethos for integrated clinical care across all care settings within Grey Bruce Health Services and with community care partners.

Clinical integration is the means to facilitate the coordination and continuous improvement of patient care across conditions, providers, settings and time, in order to achieve care that is safe, timely, effective, efficient, equitable and patient-focused.

The Director is responsible for: Patient Movement and Flow to/from, within and between all GBHS care settings; the operational oversight of the community hospitals in Lion’s Head, Markdale, Meaford, Southampton and Wiarton; the Diabetes Education Program;  and Allied Health.

Qualifications / Skills / Abilities:


  • Masters degree in a health care or related field
  • Experience in clinical management, minimum 3-5 years, and proven track record as a health care leade
  • Exceptional Leadership competencies with strong interpersonal skills, administrative capabilities and pro-active skills in team building, problem solving and decision making
  • Project management skills and use of project management processes and tools
  • Effective organizational skills and ability to work under pressure/meet deadlines
  • Highly skilled facilitator who is able to bring diverse people/teams together to work on complex issues
  • Experience leading large projects and initiatives and achieving positive results
  • Highly results driven and focused on meeting targets and timelines
  • Experience using a variety of QI tools and methods such as LEAN, SWOT analysis, PDSA, gap analysis, need assessments, etc.
  • Ability to prioritize and manage time and resources
  • Strong program planning and evaluations skills
  • Overall commitment to the quality of patient care and experience
  • Visionary and creative problem solver, with ability to make decisions and execute effectively
  • Ability to interact effectively with internal and external stakeholders at all levels along with the ability to collaborate with other regional healthcare partners and build business relationships
  • Ability to work collaboratively in a multidisciplinary team environment and to motivate a team of diverse professionals in achieving desired results
  • Strong presentations and report writing skills
  • Demonstrated effective oral and written communication and customer service skills
  • Computer skills in MS Office
  • Commitment to continuous professional development and continuing education
  • Available for regular assignment of Hospital Administrator on-call responsibilities (assigned on a rotating basis to cover evenings and weekends)
  • Valid (class G) drivers license, Ability to travel, reliable and insurable vehicle
  • Recent satisfactory performance and attendance records
  • Knowledge of and adherence to GBHS ‘CARE’ Behaviours (Collaboration, Accountability, Respect, Excellence)
  • Advocates and supports a culture of patient safety and demonstrates an understanding of how the department service and assigned responsibilities contribute to overall patient safety at GBHS through patient safety knowledge, skills and attitudes
