Grey Bruce District Stroke Centre

Grey Bruce Health Services was designated as a District Stroke Centre (DSC) for Grey and Bruce Counties in the spring of 2002. The District Stroke Centre is a component of the Ontario Stroke Strategy. The Southwestern Ontario Regional Stroke Centre located in London supports the work of the Grey Bruce DSC.

Lessening the Gamble of Care
The DSC takes ownership over the coordination of the stroke care strategy across the continuum in Grey and Bruce counties including closing gaps in the care continuum. Involvement may be direct, facilitative, supportive or consultative. In 2000-2001, 496 individuals were hospitalized for stroke or transient ischemic attacks in the Grey and Bruce counties. It is estimated that another 473 individuals had a stroke or transient ischemic attack but were not admitted to hospital. The DSC is developing and implementing strategies to organize stroke care across the continuum from prevention to acute care, rehabilitation, community reengagement and long term care.

Role of the District Stroke Centre
- To provide organized stroke care to patients in Grey and Bruce Counties
- To provide coordinated stroke service based on best practice and evidence
- To work in coordination with the Regional Stroke Centre and other partners
- To provide leadership to achieve coordinated stroke care throughout Grey and Bruce Counties and all points in the spectrum of stroke care – promotion, prevention, acute care, rehabilitation and community.
- To participate in ongoing education/training in stroke care

District Stroke Centre Personnel
The District Stroke Centre is a component of the Rehabilitation and Complex Continuing Care Program. The DSC working group is comprised of local and regional partners in stroke care.

These partners represent stroke care across the continuum and our geographic area. The DSC has a dedicated advance practice nurse and district coordinator. The Medical Champion provides guidance and advice. As the strategy proceeds additional personnel will be recruited with the implementation of the Secondary Stroke Prevention Program.
