Registration – Once you arrive at hospital

You must register for every hospital visit, and we are open 24 hours a day, 7 days a week. Depending on the date, time and reason for your visit you will be directed to the appropriate area when you arrive for your appointment.  We have 2 areas for registration at each hospital. In Owen Sound you will be directed to either Registration or our Emergency Department, and at the other locations, the Business Offices or the Emergency departments.

At Grey Bruce Health Services we are very aware that we need to capture all of your personal and health information correctly so that we can provide you with the best care possible.

When you come to register we will ask you for your Ontario Provincial Health Card along with your name, date of birth, address(es), phone numbers, emergency contact and next of kin and their contact information.  We will also ask for the name of your family doctor and/or Nurse Practitioner or Primary Care Provider. As well, we will need any Workplace Safety and Insurance Board (WSIB) information if your appointment is related to a work place incident. If you have additional health care insurance it would be helpful if you could supply us with your group benefit and Policy number.
